Understanding the price columns

The gross charge is what a hospital lists before any discounts. Most patients pay significantly less. It is essentially a starting point for negotiations between hospitals and insurers.

Cash/self-pay prices are the discounted rates for uninsured patients, typically 32-50% of the gross charge. In Houston, cash prices for a brain MRI range from $1,335.5 to $1,667.36.

Insured rates depend on your specific plan and the rate your insurer has negotiated. As the payer data below shows, these can vary enormously.

Multiple codes on one visit

It is also important to understand that these prices reflect a single HCPCS billing code. If your physician orders an MRI with and without contrast during the same visit, the hospital will bill two separate codes, and your total facility fee will be higher than the figures shown here.

High-deductible plan considerations

For patients with high-deductible health plans, the negotiated rate (not the gross charge) is the amount that applies toward your deductible. This means the "insured rate" column is often the most relevant number when estimating your actual out-of-pocket cost.

Facility type and what is included

Academic medical centres like Houston Methodist and Memorial Hermann TMC have different pricing structures than community hospitals.

Facility fees (shown here) cover the MRI machine, technologist, and overhead, but do not include the radiologist's reading fee, which is billed separately.

Freestanding imaging centres (not in our current hospital-only data) often offer lower facility fees for non-urgent scans.

Billing codes and contrast

The type of MRI matters for pricing. A brain MRI (HCPCS 70551) and a lumbar spine MRI (HCPCS 72148) are billed under different codes and carry different prices, as the tables above show. Other common MRI scans, such as knee MRI (HCPCS 73721) or shoulder MRI (HCPCS 73221), have their own separate billing codes and price ranges.

Contrast vs. without contrast also affects the total cost. An MRI "without contrast" is a standard scan. When a physician orders an MRI "with contrast," a gadolinium-based dye is injected intravenously to make certain tissues more visible. The contrast agent adds a separate charge, and the scan itself is billed under a different HCPCS code (for example, 70553 for brain MRI with and without contrast). Contrast MRIs are typically ordered when the doctor needs more detail, such as for tumors, infections, or inflammatory conditions.

When doctors order MRIs

When doctors order MRIs: Physicians typically order MRI scans after initial evaluation with physical exams or X-rays has not provided a clear diagnosis. Common reasons include persistent back pain lasting more than six weeks, neurological symptoms like numbness or weakness, suspected joint injuries such as torn ligaments, and screening for certain cancers. Many insurance plans require prior authorization before approving an MRI. This requirement can affect both scheduling and out-of-pocket costs.
